Please check each box indicating your understanding that you must complete all of the following individual requirements before the Petition for Charter is submitted in order to be activated.

I must be in good standing with the university.
I must uphold the standards of Omega Phi Alpha.
I must participate in at least five prospective new chapter-planned service projects, earning at least one hour of service in each.
I must complete at least ten hours of service through prospective new chapter-planned projects.
I must participate in at least one prospective new chapter fundraiser.
I must attend and participate in the prospective new chapter retreat, to be held midway through the expansion period.
I must complete five membership quizzes and pass with a combined average of 85%. These must be taken before the national membership test.
I must successfully pass the national membership test with a score of 85% or better (68 out of 80 questions).
I must join at least one chapter planning committee.
I must attend weekly prospective new chapter meetings. I may miss up to three meetings in a semester.
I must meet all local and national financial requirements, including but not limited to: Paying the expansion fee ($75), Paying any local dues set by the prospective new chapter, Paying for any additional activities set by the prospective new chapter

I understand that I am required to pay $75 in expansion fee. This amount is non-refundable should I choose to myself from the expansions process or not become an active member of Omega Phi Alpha.
I have read Omega Phi Alpha’s Policies (Drug and Alcohol, Hazing, Non-Discrimination) and I understand I will be asked to remove myself from the expansions process if I am not in compliance.
I understand the Hazing Policy and will notify the Expansions Director if I feel I have been treated unfairly.
I understand that as a prospective new member, I may not wear the sorority’s Greek letters, or obtain any personal sorority memorabilia with Greek letters on it until my induction as an active member.
I understand that I may voluntarily remove myself from the expansion process at any time. I must contact the prospective new chapter president and notify them of my decision.
If I am removed from the expansion process or remove myself, I may join Omega Phi Alpha on this campus at a later date once the prospective new chapter has been inducted. I may not re-enter the expansion process as a prospective new member.
If I complete all the above-listed requirements in addition to my prospective new chapter completing all requirements, I shall be inducted as an active member of Omega Phi Alpha. I will be required to attend an induction ceremony at the

If I have not met a requirement but steps can be made to resolve the deficiency, I will do so.
If I do not meet a requirement that cannot be rectified (i.e. not passing the national test, not attending the prospective new member retreat), I will be removed from the expansion process immediately.
